Illness Policy

If you have any questions about our illness policy, you can see this attachment.  This policy applies to all of our children who attend on a regular basis, as well as our children that are just here for drop-in days.  If you have any questions, you can call us.

1095-B Form

 Form 1095-B provides information about health insurance coverage. Our organization is not required to automatically send Form 1095-B to individuals.
